If you have already submitted an online claim and would like to make changes to your filed claim, please contact the Settlement Administrator.
If you wish to file a claim online, you may do so by clicking CONTINUE. Please note that the online claim will not be deemed complete unless you,
read and accurately complete all sections of the Claim Form, upload supporting documentation,
and click the "submit" button at the end of the process. Once you have entered the claimant identification information,
you will be provided a username and password. Your username and password will be available for use until you complete the online filing process.
Once you hit "submit" at the end of the process, the username and password will no longer be available. If you wish to make any changes to the claim after submission,
please contact the Settlement Administrator via telephone 1-833-380-5570 or email firstname.lastname@example.org.
